They are used in conjunction in exteriors. In interiors, the EnvMap setting applies the stock reflection map:

Yes. You can replace the stock maps, obviously, to achieve custom reflection effects but this will then apply the changes to everything using the EnvMap or EnvMap2 setting.
As for applying new custom ones, I don't think its possible. I haven't been able to isolate the setting in the nif that references the maps themselves - the EnvMap settings seem hard coded to use the existing maps and simply naming it EnvMap_something else and making new maps does nothing at all.
I don't know enough about decoding the nif but the NifTools team don't seem to have an answer either.
